Please join the Oregon Gerontolo gical Association for a presentation about the Oregon Advance Directive and POLST. This will be an update on how to document the decisions that you make in your Advance Health Care Planning. The Oregon Advance Directive and the Oregon POLST both appeared in 1993. Ever since, many Oregonians ha ve been confused about which document best supports their wishes and intent ions. 28 years later in 2021, the Oregon Legislature made the first signifi cant update to the Advance Directive for the purposes of (1) clarifying the unique roles of the Advance Directive and the POLST and (2) updating the A dvance Directive to work more effectively with current medical practice. Ac cordingly, the Advance Directive has been completely rewritten. In this ses sion, we will review the new Advance Directive and answer your questions ab out it.
By the end of this prese ntation, we hope you will:
1. Kn ow why the need for urgency has shaped the unique structure for documenting Advance Health Care Planning in Oregon
3. Know whe n to request the Oregon POLST
4. Know how the Advance Directive and POLST work together to support your Adv ance Health Care Planning

Presenters:
Wo ody English, MD is the Co-Chair of the Oregon Advance Directive Ad visory Committee. His medical training has included Internal Medicine, Infe ctious Diseases, Epidemiology, Public Health, Hospital Medicine, Palliative Care, and Hospice. He has worked in Oregon since 1977. In 2007 he co-found ed the palliative care service for Providence and retired from the practice of medicine in 2013. Since then, he has worked with various committees to update the Oregon Advance Directive, which was most recently revised on Jun e 2, 2021 with the passage of Senate Bill 199. Additionally, he participate s in several committees for the Oregon POLST Coalition and serves on the Bo ard of Directors for CareOregon.
Eriko Onishi, MD, MCR, PhD graduated from Tsukuba School of Medicine in Japan. She came to the US
to obtain Family Medicine (FM) Residency training, after which she practice
d in a community in rural Indiana until joining the Oregon Health and Scien
ce University (OHSU) Department of Family Medicine in October 2011. Since 2
011, Eriko has been practicing FM both at OHSU and as part of the Palliativ
e Care Medicine team at Salem (Oregon) Hospital. She is also the Medical Di
rector of a Portland area nursing home. Eriko has been serving as a member
of the State of Oregon Advance Directive Adopted Committee since April 2019
, where she and the other committee members have worked to revise the Orego
n state Advance Directive (AD) form and are working to educate Oregonians a
bout Advance Care Planning and the revised Oregon AD. At OHSU, Eriko is an
Assistant Professor at the OHSU Department of Family Medicine and a Clinica
l Associate Professor of General Medicine at Tsukuba School of Medicine, wh
ere she works to teach and train FM residents and medical students in the U
S and Japan.
